Coming into the Monash Health TMHN graduate program, my overall objective was to develop my mental health nursing skills from medication administration; mental state examinations; communication, including conducting clinical handovers, therapeutic engagement and de-escalation skills; time management; treatment care planning through to self-awareness and self-care. After 12 months of undergoing the TMHN program, I can safely say that I have come a long way in achieving this objective, and look forward to continuing professional development. The year was both eye-opening and a success for me in many respects. It exposed me to the diversity of mental health conditions and behaviours, and accorded me an opportunity at the same time to learn how to manage these conditions and behaviours. It opened my eyes to the magnitude of substance abuse problem facing many people within and without the Monash Health catchment area, and the negative impact it is having on both patients and their families (or carers). It increased my awareness of the provisions of the Mental Health Act (2014) and its recovery-oriented approach to mental health in light of the travesties of the past, traditional custodial, medical-oriented approaches to mental health, including institutionalisation. It awakened my consciousness regarding the values of care, including empathy, respect, acceptance, confidentiality, and integrity. Also, the program provided me with an opportunity to work with truly incredible people and bear witness to the power of team work and team spirit. However, this year-long journey has not been without its challenges. Some of the major challenges that I encountered include communication problems not only with patients, but also with staff. These problems were particularly challenging partly because of they are deeply rooted in 'ward' (aka organisational) culture, and partly due to my personal inexperience dealing with challenging communication situations. However, as I go into the next stage of my career as a mental health nurse, I am looking forward to continue my professional development, polishing up my skills, and providing evidence-based best practice to achieve optimum patient outcomes.
