Paliperidone (Depot)
a) Is the medication a typical/atypical antipsychotic?
:: Atypical.
:: Used for the treatment/management of schizophrenia or acute exacerbations of schizoaffective disorder.
b) Common brand names.
:: Invega Sustenna.
c) Normal dose range.
:: 25-150mg.
:: Clients generally start on a dose of 150mg on day 1, then 100mg on day 8, then 75mg a day later (which is the maintenance dose). A maintenance dose once a month is then administered which ranges from 25-150mg.
d) COMMON side effects.
:: Akathisia- motor restlessness (usually occurs 2-3 days, up to several weeks) after first commencing treatment. Akathisia generally improves with dose reduction.
:: Tachycardia- increased HR.
:: Injection site reactions- such as swelling, pain and redness.
e) Contraindications.
:: Hypersensitivity reactions: anaphylactic reactions/angioedema.
:: Paliperidone is a metabolite of risperidone so it is contraindicated if the client is hypersensitive to risperidone.
- Must be used in caution if the client has:
:: Seizures.
:: Kidney/Liver issues.
:: Diabetes.
:: Low white blood cell count.
:: Cardiovascular issues.
f) Special monitoring/nursing care requirements.
:: Educate patient on medication (including possible side effects to look out for).
:: Ensure that administration is correct (in deltoid/gluteal muscle), ensuring to zed track and draw back prior to administrating the drug.
:: Check reaction to medication if it’s the client’s first time.
:: Check for common side effects listed above, as well as rare side effects (sleep disturbance or increased salivation).
:: Check for signs of NMS, EPSE, metabolic changes (hyperglycaemia, weight gain) and hyperprolactinemia.
:: Monitor for clinical improvement.
:: When switching from one antipsychotic to another, it is important to closely monitor the client.
:: Ensure the client discloses any use of other medications, including herbal treatments.
g) Education requirements for patients/families/carers.
:: Ensure that the patient/family has been educated about purpose of medication.
:: Ensure that the patient/family is aware of potential side effects that may arise.
:: Ensure that the patient/family know that it is important to report any changes in mental state (improvement/deterioration) and possible side effects they are experiencing.
:: Ensure that the patient/family understand that it is vital for the client to stay away from illicit substance use as they can cause a decrease in control of psychotic symptoms as well as, increase the risk of relapse.
